2010/11-2014-15 CITY OF SANTA ANA CONSOLIDATED PLAN I 2012/13 <br />ACTION PLAN <br />¦ Consultation with Continuum of Care (CoC) - The City must consult with the CoC that <br />serves the regional homeless <br />¦ The HEARTH Act also required changes to the way ESG funds are allocated; specifically, <br />only 60 percent of the City's ESG grant may be used for shelter and outreach services <br />The Act also modified the definition of homelessness and allows ESG grantees to use some <br />Continuum of Care funding for people who are not homeless under HUD's definition, but <br />are homeless under definitions of homelessness used by other federal agencies. <br />Additionally, the HEARTH Act added a definition for a person that is at imminent risk of <br />homelessness or where a family or unaccompanied youth is living unstably. The Hearth Act <br />also required that the City make certain modifications to, its Consolidated Plan, Action Plan <br />and year-end performance reports. Amendments, to the Consolidated Plan will be <br />undertaken by the City as part of a separate action. The 2012-2013 Action Plan will take <br />into account required modifications as will the upcoming 2011-2012 Consolidated Annual <br />Performance and Evaluation Report (CAPER, i.e., year-end performance report). Taking the <br />HEARTH Act into account, during the 2012-2013 Program Year, the City will undertake the <br />following homeless housing/service activities: <br />HOMELESS ASSISTANCE - OUTREACH & SUPPORTIVE SERVICES <br />Santa Ana's Continuum of Care strategy calls for the provision of outreach and support <br />services to assist individuals and families that are homeless or at risk of becoming homeless. <br />ESG resources will be provided to local nonprofit organizations and the City's police <br />department; outreach ':services will be provided to an estimated 947 individuals during FY <br />2012-2013. Additionally, "the City has allocated CDBG funds to fund the HEART Program, <br />which will be used to supplement ESG funding for outreach/assessment. These activities <br />are summarized in EXhlbit 3 - LISTING OF PROPOSED PROJECTS. Additionally, on a countywide <br />basis, HOPWA will be used to provide homelessness supportive services to an estimated <br />376 households. <br />HOMELESSNESS PREVENTION <br />A key component of Santa Ana's Continuum of Care strategy is to prevent individuals and <br />families from falling into homelessness. This is accomplished by providing monetary <br />assistance and counseling/support services to households at risk of becoming homeless. <br />The City's continuum of care for the homeless strategy calls for providing ESG resources to <br />agencies that provide this type of assistance. An estimated 288 individuals will be assisted <br />with homelessness prevention services during FY 2012-2013. Specific activities to be <br />undertaken during the 2012-2013 Program Year are outlined in the LISTING OF PROPOSED <br />PROJECTS - EXHIBIT 3.
