INSURANCE ON FILE <br />WORK MAY PROCEED N- 2010 - 058 -002 <br />UNTIL INSURANGE EXPIRES <br />CLERK OF COUNCI <br />DATMA� �� g 2b12 SECOND AMENDMENT TO AGREEMENT <br />THIS SECOND AMENDMENT TO RECREATION SERVICES AGREEMENT is made and <br />entered into this 26`h day of April, 2012 by and between Blanca Arceo, (hereinafter "Provider "), <br />R and the City of Santa Ana, a charter city and municipal corporation organized and existing under <br />� N the Constitution and laws of the State of California (hereinafter "City "). <br />�"- U <br />N <br />:� RECITALS: <br />N 1. The parties entered into Agreement N- 2010 -058, dated July 1, 2010, as amended from <br />O <br />time to time, (hereinafter the "Agreement ") by which Provider has provided recreation <br />and leisure class services in the City's leisure program. <br />2. The parties entered into First Amendment to Agreement N -201 0 - 081 -001, dated April 7, <br />201 1, amending the Compensation and Term of the original Agreement. <br />3. In accordance with the terms and conditions of said Agreement, the parties wish to revise <br />the Scope of Services (Exhibit A), and extend the term. <br />WHEREFORE, in consideration of the covenants contained in said Agreement, and subject to all <br />the terms and conditions of said Agreement, except those amended in this Second Amendment to <br />Recreation Services Agreement, the parties agree as follows: <br />1. Section 1, SCOPE OF SERVICES, shall be amended by substitution of a new Exhibit A, <br />attached hereto and incorporated by reference. <br />2. Section 3, TERM, shall be amended to extend the termination date to June 30, 2013. <br />3. Except as herein amended, all terms and conditions of said Agreement shall remain in full <br />force and effect. <br />IN WITNESS WHEREOF, the parties hereto have executed this Second Amendment to <br />Recreation Services Agreement on the day and year first set forth above. <br />ATTEST: C[TY OF SANTA ANA <br />,��v <br />Mana D.
