INSURANCE ON FILE <br />WORK MAY PROCEED N-2010-028-002 <br />UNTIL INSURANCE EXPIRES <br />i,e-ao-,- a <br />CLERK OF COUNCIL <br />CLJUN 6 Zat2 <br />SECOND AMENDMENT TO AGREEMENT <br />O: PRCS <br />Silvia Cue2&s SECOND AMENDMENT TO RECREATION SERVICES AGREEMENT is made and <br />entered into this 26"' day of April, 2012 by and between Nydia Duras(hereinafter "Provider"), <br />and the City of Santa Ana, a charter city and municipal corporation organized and existing under <br />the Constitution and laws of the State of California (hereinafter "City"). <br />RECITALS: <br />The parties entered into Agreement N-2010-028, dated March 1, 2010, as amended from <br />time to time, (hereinafter the "Agreement") by which Provider has provided recreation <br />and leisure class services in the City's leisure program. <br />The parties entered into First Amendment to Agreement N-2010-028-001, dated May 12, <br />2011, amending the Scope of Services and Term of the original Agreement. <br />In accordance with the terms and conditions of said Agreement, the parties wish to revise <br />the Scope of Services (Exhibit A), and extend the term. <br />WHEREFORE, in consideration of the covenants contained in said Agreement, and subject to all <br />the terms and conditions of said Agreement, except those amended in this Second Amendment to <br />Recreation Services Agreement, the parties agree as follows: <br />1. Section 1, SCOPE OF SERVICES, shall be amended by substitution of a new Exhibit A, <br />attached hereto and incorporated by reference. <br />2. Section 3, TERM, shall be amended to extend the termination date to June 30, 2013 <br />3. Except as herein amended, all terms and conditions of said Agreement shall remain in full <br />force and effect. <br />IN WITNESS WHEREOF, the parties hereto have executed this Second Amendment to <br />Recreation Services Agreement on the day and year first set forth above. <br />ATTEST: <br />Crti.� <br />Maria 17.
