ARTICLE XII <br />12.0 EMPLOYEE INSURANCE <br />12.1 During the term of this Agreement the City will contribute toward the payment of premiums <br />for health, dental and long-term disability insurance plans administered by the Association <br />for the benefit of the employees represented by the Association as follows: <br />A. Medical Insurance. During the term of this Agreement, the City will contribute the <br />following amounts to the Santa Ana Police Officers Association Medical Insurance <br />Trust Fund for each affected employee enrolled in medical insurance plans provided <br />by the Association for its bargaining unit members and their eligible dependents: <br />Effective 1-1-07: $767/month per employee (represents an 8% increase). <br />Effective January 1, 2008, January 1, 2009, and January 1, 2010, respectively, the <br />City shall contribute toward medical premiums an amount equal to the percentage <br />increase associated with the Kaiser California CaIPERS HMO plan. Any <br />contributions necessary to maintain benefits under said medical plans in excess of the <br />amounts referenced shall be borne entirely by the Association and/or the enrollees. <br />Effective January 1, 2011, January, 1 2012 and January 1, 2013, respectively, the City <br />shall contribute toward medical premiums an amount equal to the percentage increase <br />associated with the Kaiser California CaIPERS HMO plan. Any contributions <br />necessary to maintain benefits under said medical plans in excess of the amounts <br />referenced shall be borne entirely by the Association and/or the enrollees. <br />B. Dental Insurance. During the term of this Agreement, the City will contribute the <br />following amounts to the Santa Ana Police Officers Association Medical Insurance <br />Trust Fund for each affected employee enrolled in dental insurance plans provided by <br />the Association for its bargaining unit members and their eligible dependents: <br />Effective 1-1-06 thru 12-31-09: $ 80/mo per employee. <br />Effective 1-1-10: $90/mo per employee. <br />Any contributions necessary to maintain benefits under said dental plans in excess of <br />these amounts per month shall be borne entirely by the Association and/or the <br />enrollees. <br />C. Long-Term Disability (L.T D) Insurance. The City shall continue to contribute to the <br />Santa Ana Police Officer Medical Insurance Trust Fund an amount toward the <br />payment of premiums for each employee who is enrolled in the long-term disability <br />insurance plan provided by the Association for employees covered by this Agreement <br />in accordance with the following schedule: <br />1. With respect to sworn, "safety-member" police personnel, up to a maximum <br />50 <br />
