EXHIBIT A <br />SCOPE OF SERVICES —Maria Ruiz /Preschool <br />1) Provider will teach Preschool Program classes for children ages 3.5 - 4yrs, 9 months <br />old. The program will help children grow to be independent, able to work well with <br />others and gain teamwork skills. <br />2) Classes shall be 3 hours, four days a week, conducted in one -month sessions during <br />the months of August through June, November and December shall be three -week <br />sessions. <br />3) Provider shall work with City staff in setting a schedule for classes, including the <br />location, days and hours when class will be held and holidays to be observed. <br />4) Provider shall provide materials, supplies, equipment, records and personnel. Provider <br />shall be responsible for clean-up of the facilities and materials and shall ensure the <br />safety and effectiveness of instruction. <br />5) If Provider allows others to teach class, those teachers must be over 21 , have obtained and <br />maintain an instructor rating, and be covered by Providers' insurance. Provider shall provide <br />City with documentation to verify instructor and insurance requirements. <br />CLASS SIZE <br />1) Each class must have a minimum of 18 paid students and a maximum of 25. <br />2) No registration will be accepted after the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the first class, the <br />class may be cancelled by mutual agreement of Provider and City. In such event, no <br />compensation shall be owed to Provider. <br />CLASS FEES / <br />�v Yom' <br />1) Each participant shall pay the class registration fee of $per th or $25 per week per <br />participant. An additional $10 material/supplies fee will be paid by participants directly to the <br />instructor on the first day of class. <br />2) No refunds will be made to participants after the first week of class unless the class is <br />cancelled by the City. <br />3) The City shall collect registration fees from each participant in the class during the <br />registration period. Provider shall not collect registration fees, but shall refer all interested <br />participants to City for registration. <br />4) Provider shall receive eighty percent (80 1/6) of the total fees collected each month. City and <br />Provider agree that City shall retain twenty percent (20 %) of the fees collected as an <br />administration fee_ <br />5) Provider agrees that City is entitled to audit Provider's records and classes to insure <br />compliance with this Agreement. <br />6) Provider may not waive class participation /registration fees. <br />7) Only registered participants may participate in class. <br />
