SIXTH AMENDMENT TO AGREEMENT <br />THIS SIXTH AMENDMENT TO AGREEMENT is entered into on this _ day <br />of , 2012 by and between Civic Collections, Inc., a California <br />corporation ("Contractor") and the City of Santa Ana, a charter city and municipal <br />corporation of the State of California ("City"). <br />Recitals: <br />A. The parties entered into Agreement #N-2008-141, dated October 15, 2008, <br />(hereinafter "said Agreement") by which Contractor has provided dog license fee <br />collection services. <br />B. The parties entered into First Amendment to Agreement #A-2008-292, dated <br />November 17, 2008, in order to adjust the compensation of said Agreement. <br />C. The parties entered into Second Amendment to Agreement #A-2009-063, dated June <br />1, 2009, in order to adjust the compensation and extend the term of said Agreement. <br />D. The parties entered into Third Amendment to Agreement #A-2010-117, dated July 6, <br />2010, in order to extend the term of said Agreement. <br />E. The parties entered into Fourth Amendment to Agreement #A-2011-183, dated May <br />19, 2011, in order to adjust the compensation and extend the term of said Agreement. <br />F. The parties entered into Fifth Amendment to Agreement #A- 2011-183-01, dated <br />July 18, 2012, to amend the compensation of said Agreement to pay for additional <br />services that were provided during the term of said Agreement. <br />G. Thereafter, upon the expenditure of the allocated funds, the Parties agreed to increase <br />the compensation to pay for additional services provided by Contractor during the <br />term of said Agreement. However, due to an oversight and invoicing error, this <br />agreement was not formally executed in writing, although Contractor provided <br />additional services during the term set forth in said Agreement. <br />H. In accordance with the terms and conditions of said Agreement, the parties wish to <br />amend the compensation of said Agreement to pay for additional services that were <br />provided by Contractor during the term of said Agreement. Per agreement of the <br />parties, this Sixth Amendment to Agreement is now made to include all services <br />previously provided by Contractor to City in relation to the collection of dog license <br />collection services. <br /> <br /> <br />25C-5
