¦ Mental Health Association of Orange County - worked with the City's chronic homeless <br />population, providing services and access to housing from a number of "street" locations <br />throughout the City. <br />¦ Mercy House staffed the Winter Armory Shelter program, provided night-time shelter, <br />meals, and referral services to individuals living on streets, parks, cars, etc. <br />¦ Veteran's First provided transitional housing and support services and housing for homeless <br />veterans. <br />¦ Orange County Health Care Agency - operated a proactive outreach program to assist <br />homeless persons with HIV/AIDS living on the streets. A portion of this program was <br />funded with HOPWA. <br />¦ HOPWA Tenant Based Rental Assistance - Funding to assist individuals living with HIV/AIDS <br />maintain independent living situations. <br />Santa Ana did not apply for Continuum of Care Super NOFA funds in 2011 or 2012. No requests for <br />Certification of Consistency with the City's Consolidated Plan were received by the City from NOFA <br />applicants. The City of Santa Ana did participated in the development of the regional Continuum of <br />Care strategy by providing valuable mainstream fund information and assigning key staff to <br />participate in the Orange County Continuum of Care Community Forum. The Community Forum <br />was responsible for identifying the gaps and priority needs for the County's 2011 and 2012 <br />associated application for Super NOFA homeless funds. <br />08/23/2012 21 <br />19C-29
