C. PART 1-13. ANNUAL PERFORMANCE UNDER THE ACTION PLAN <br />1. Summary of Housing Activities: <br />a. Supportive Housing Activities: <br />During 2011-2012, the following supportive housing accomplishments were achieved <br />through the expenditure of HOPWA funds: <br />¦ 9 individuals received treatment via the Medical/Social model residential detoxification <br />program <br />¦ 447 individuals were provided with supportive housing for substance abusers <br />b. Housing Assistance for the Homeless: <br />¦ 70 individuals/families received short-term emergency financial assistance to prevent <br />homelessness <br />¦ 130 homeless individuals received short-term (emergency shelter) and transitional <br />housing services. <br />¦ 50 individuals/families were provided with permanent housing rental assistance <br />c. Other housing activities: <br />¦ NA <br />2. Progress Evaluation: <br />Overall, and especially in the context of a continuum of housing, 2011-2012 HOPWA projects met <br />or exceeded planned goals.
