A-2012-095 <br />SECOND AMENDMENT TO <br />CONSULTANT AGREEMENT <br />THIS AMENDMENT, made and entered into this 215t day of May, 2012 by and between <br />Scott Weldy DVM, dba Serrano Animal & Bird Hospital (hereinafter "Consultant"), and the City <br />of Santa Ana, a charter city and municipal corporation organized and existing under the <br />Constitution and laws of the State of California (hereinafter "City"). <br />RECITALS <br />A. The parties entered into Agreement 4 A-2010-083, (said "Agreement") by which <br />Consultant has provided veterinarian services to the City. <br />B. By Amendment A-2011-124, dated May 2, 2011, the parties extended the term and <br />provided compensation to pay for services during the extended term. <br />C. The parties wish to again amend the Agreement to modify the Compensation and Term. <br />WHEREFORE, in consideration of the covenants contained in said Agreement, and subject to all <br />the terms and conditions of said Agreement, except those amended in this Amendment, the <br />parties agree as follows: <br />1. Section 1, SCOPE OF SERVICES, Consultant will continue to provide those services for the <br />Santa Ana Zoo at Prentice Park as set forth in Exhibit A, attached hereto and incorporated by <br />this reference. <br />2. Section 2.a., COMPENSATION, shall be amended to provide an additional $65,000 to pay <br />for services provided during the extended term of said Agreement, at the rates and charges set <br />forth in Exhibit A. A maximum amount of $10,000 shall be available for reimbursable <br />expenses, for a total not to exceed amount of $75,000 available during the term from August <br />1, 2011 through July 31, 2012. <br />3. Section 3, TERM, shall be amended to extend the term for an additional one-year period, <br />through July 31, 2013, unless terminated earlier in accordance with the terms of said <br />Agreement. <br />3. Except as hereinabove amended, all terms and conditions of said Agreement shall remain in <br />full force and effect. <br />/// <br />
