Carvalho <br />CLERK OF THE COUNCIL <br />Maria Huizar <br />Pursuant to Agreement A- 2012 -075, entered between CARE Ambulance Service, Inc. and the <br />City of Santa Ana, dated April 2, 2012, Section 5 - Term, the time period of said Agreement is <br />hereby extended for an additional thirty (30) day period, through September 19, 2012. The <br />insurance certificates and Additional Insured Endorsement are required to be extended and /or <br />renewed to cover this extension.
