<br />City of Santa Ana <br />Scope of Work <br />Name of Organization Human Options <br />Name of Funded Program Human Options Emergency Shelter <br />Annual Accomplishment Goal <br />1. Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12-month contract period. <br />215 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br />contract period. <br />23 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />ESG funds will be used specifically to provide: <br />1. Case Management services to Santa Ana shelter clients <br />2. Funds will be used to cover utilities at the shelter <br />3. Funds will be used to maintain and repair the shelter <br />4. Funds will be used to purchase food for Santa Ana shelter clients <br />Human Options Emergency Shelter provides safety and services to victims of domestic violence. The shelter <br />offers a wide range of on-site essential services which address health, drug abuse, domestic violence, <br />education, counseling, and housing. A Case Manager is assigned to each woman who enters the shelter. The <br />Case Manager will meet iwt hthe individual 1-2x/week and will assist her in obtaining transitional or permanent <br />housing; medical services; substance abuse treatment and counseling; financial assistance, child case, and <br />transporation assistance. Each woman is assigned a counselor who provides psychological counseling to help <br />her with her emotional needs and assists her in recovering from the trauma of domestic violence. The <br />counselor also provides family counseling 1x/per week to woman and their children. All women particpate in <br />psycho-educational, process oriented, and life skills groups. These groups includ psychoeducation about <br />domestic violence, parenting, safety planning building healthy relationships, drug and alcohol abuse, and <br />financial literacy. Human Options ER Shelter additionally provides legal support. Our legal advocates assist <br />each client according to her identified needs. A legal advocate can assist in applying for a restraining order, <br />accompaniement to court, and providing legal linkages and referrals for divorce and custody issues. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
