City of Santa Ana <br />Scope of Work <br />Name of Organization Orange County Association for Mental Health dba MHA <br />Name of Funded Program City of Santa Ana Outreach <br />Annual Accomplishment Goal <br />1. Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12-month contract period. <br />Persons <br />II. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What speck activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />-he primary goals of the Program are to first locate, then engage the mentally ill homeless of Santa Ana and <br />ssess their social, mental and medical needs. Secondly, Program staff will assist Consumers in accessing the <br />hental health, social services and medical services they need to improve their situations and end their <br />omelessness. An important but tertiary goal is to provide a mental health expertise and network to services <br />ffered by existing homeless service providers in the City. The Outreach Team will engage 600 unduplicated <br />omeless individuals in the city of Santa Ana over the course of the program year and provide at least 2400 <br />nits of services to those involved with the Program. Of the 600 unduplicated homeless individuals in the City <br />f Santa Ana, 250 will be actively linked to the psychosocial, mental health and medical services they need to <br />egin the process that will end their homelessness and provide them a more successful and satisfying life. <br />Vithin the City of Santa Ana we will provide a half time Spanish-speaking Outreach Worker to the various <br />xisting sites providing services to the homeless and low income individuals to assess for signs of mental <br />Iness and to act as the port of entry process to obtaining mental health services. Outreach will be provided by <br />his bi-lingual specialist counselor exclusively to the City of Santa Ana and will include street outreach, ride- <br />long with Santa Ana Police, in addition to outreach to local homeless providers and shelters. There are a <br />pecific number of scheduled sites within the city of Santa Ana upon approach the Outreach Worker will be <br />vailable at specific locations certain days and times. The OW will approach individuals existing in the program <br />) update themselves on how the individual is doing on that particular day and what the needs are. Support will <br />e given along with additional resources and assessment. New individuals will be approached and the <br />ngagement assessment process will begin <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
