Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Representation, legal pleading preparation, counsel and advice, or education to at least 78 income qualified <br />Santa Ana households who have been served with an Unlawful Detainer or a notice of eviction. <br />2. Priority representation to at least 10 clients of other ESG service providers who need help enforcing or <br />creating a settlement agreement or other legal assistance to prevent homelessness. <br />Rental Assistance: <br />3. Rental assistance for at least 5 clients who meet the following criteria: <br />a. Served with an unlawful detainer for non-payment of rent; <br />b. Non-payment of rent was caused by an unexpected crisis; <br />c. Client does not have sufficient resources or support networks immediately available to prevent them from <br />moving to a shelter or becoming homeless; <br />d. Client owes a maximum of three months rent; <br />e. Client's monthly rent is equal to or less than 60% of their verifiable household monthly income; <br />f. The unexpected crisis that caused the non-payment of rent has ended and indications are that they will be <br />able to stay stably housed for the next six months after the payment of rental assistance; and <br />g. Client meets all other ESG requirement homelessness prevention assistance requirements <br />CMIS data entr : <br />4. Staff will be entering essential client data into CMIS as they enter our program and updating housing status <br />as they completely exit our program. PLC has budgeted $250 per month for CMIS data entry which is <br />about $62.50 per week funding approximately 4 hours of staff time. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
