MEMORANDUM
David Cavazos,
To: City Manager
Edwin "William" Galvez, P.E. Interim
Executive Director,
From: Public Works Agency
Date: October 21, 2013
Project Nos.:137513 & 137514; El Salvador/Windsor Park Walkway
Reconstruction
Subject: Change Order No. 1
Contractor: Pacific Construction Company, Inc. o
10910 Densmore Avenue
Granada Hills, CA 91344
z> °`
Project Description: El Salvador/Windsor Park Walkway Reconstruction =
co
Change Order Number: 1 Total This Change Order $ 43,006.00
Accounting Unit - Activity Number: 13518783 - 13751301028
JUSTIFICATION
The concrete walkways at El Salvador and Windsor Parks project are funded by the
Community Development Block Grant Fund Program which was established in 1974 by
the United States Department of Housing and Urban Development and provides grant
funds for to address community development needs. However, due to funding
constraints and the engineer's estimate, certain improvements that encompassed
these requirements were deleted from the final design.
Consequently with the bids coming in lower than expected, Parks, Recreation and
Community Services (PRCS) staff recognized this opportunity to utilize grant funds to
add enhancements and requested the to rehabilitate the parking lot at south end of El
Salvador Park. This compensates the contractor for all costs related to the re-
construction of the pavement, an ADA curb ramp and restriping per City of Santa Ana
PCO #2 dated 5/7/2013.
AGREED COST = $43,006
