PERSONNEL DISCLOSURE <br />28.1 CONTRACTOR shall make available to ADMINISTRATOR a <br />current list of all personnel providing services hereunder, <br />including resumes and job applications. Changes to the list will <br />be immediately provided to ADMINISTRATOR in writing, along with a <br />copy of a resume and/or job application. The list shall include: <br />28.1.1Names of all full or part-time personnel by <br />title, including volunteer personnel, whose direct services are <br />required to provide the programs described herein; <br />28.1.2A brief description of the functions of each <br />position and the hours each person works each week; or for part- <br />time personnel, each day or month, as appropriate; <br />28. The professional degree, if applicable, and <br />experience required for each position; and <br />28. The language skill, if applicable, for all <br />personnel. <br />28.2 CONTRACTOR's employment applications shall require <br />applicants to provide detailed information regarding the <br />conviction of a crime by any court, for offenses other than minor <br />traffic offenses. Information not disclosed in the employment <br />application discovered subsequent to the hiring or promotion of <br />any applicant shall be cause for termination of that employee. <br />28.3 Where authorized by law, CONTRACTOR shall conduct, at <br />no cost to the COUNTY, criminal record background checks on all <br />employees and/or volunteers who will provide services under this <br />Agreement. Candidates will satisfy background checks consistent <br />with and comparable to those required for COUNTY employees. <br />(WAM0313) <br />35 of 48 <br />25B-37 <br />(April 8, 2013)
