Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12-month contract period. <br />1800 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br />contract eriod. <br />225 Persons <br />Proaram and Fundina Uescriotion <br />Homeless individuals are most receptive to interventions when shelter, food, clothing, and transportation needs are met <br />IF's team of compassionate case managers and health care workers engage and work closely with this population to <br />ensure that they access and follow-through with the services they need to improve their quality of life. Streettmotel <br />outreach will be conducted through neighborhood clinics held at locations where the greatest numbers of at-risk <br />individuals are known to congregate in order to reach out to as many individuals as possible. <br />;rgency Health Services: Teams of physicians, public health nurses, and mental health counselors meet with <br />tints to assess individuals, providing them with: <br />• Triage and brief assessment: health history and status <br />• Testing and examination: blood pressure screenings, blood sugar testing, etc. <br />• Primary and urgent care: respirator, gastrointestinal, vision, pain, headaches, etc. <br />• Chronic disease screening/monitoring: Diabetes, hypertension, asthma <br />• Chronic disease management and health education: obesity, hypertension, etc. <br />• Pediatrics: child examinations and evaluations <br />• Flu shots <br />• MSI enrollment <br />• Referrals to medial homes <br />Case management: Families referred from clinics, outreach events and/or programs (such as 2-1-1-) are stably housed <br />in motels. Case managers meet with clients to identify barriers to self-sufficiency and create action plans with weekly <br />goals to promote ongoing progress and improvements in children's development. Services include job and financial <br />counseling, life skills, mental health counseling. Other services not covered by ESG funding include: motel vouchers, <br />basic goods (food, clothing, diapers), and transportation (bus passes and gas cards.) <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
