Such listing shall also include the full names <br />of all of CONTRACTOR's officers, directors, partners and those holding a <br />financial interest. Included are members of its advisory boards, members of <br />its staff and consultants, who have any family relationship by marriage or <br />blood to an officer, director, or stockholder of the corporation or to any <br />partner of the partnership. In preparing the latter listing, CONTRACTOR shall <br />also indicate the names of the officers, directors, stockholders, or <br />partner(s), as appropriate, and the family relationship which exists between <br />such person(s) and CONTRACTOR's representatives listed. <br />8.3.5 True and correct copies of all agreements with respect to <br />any such real property shall be appended to the affidavit described above and <br />made a part thereof. If, during the term of this Agreement, there is a change <br />in the agreement(s) with respect to real property where persons receive <br />services, CONTRACTOR shall promptly notify ADMINISTRATOR, in writing, <br />describing such changes. <br />9. NON-DISCRIMINATION <br />9.1 In the performance of this Agreement, CONTRACTOR agrees that it <br />shall not engage nor employ any unlawful discriminatory practices in the <br />admission of clients, provision of services or benefits, assignment of <br />accommodations, treatment, evaluation, employment of personnel or in any other <br />respect on the basis of sex, race, color, ethnicity, national origin, <br />ancestry, religion, age, marital status, medical condition, sexual <br />NAM0313) <br />13 of 40 <br />(April 8, 2013)
