$7.10 for each Primary Enrollee without enrolled Dependents; and <br />$17.49 for each Primary Enrollee with one or more enrolled Dependents. <br />The Contractholder is required to pay only the billed, monthly Premiums shown above. The total <br />monthly Premium, including billed and subsidized monthly Premiums, is: <br />$55.49 for each Primary Enrollee without enrolled Dependents; and <br />$136.66 for each Primary Enrollee with one or more enrolled Dependents. <br />Contractholder agrees to pay the invoiced amount. Eligibility adjustments reported to Delta <br />Dental after the date the invoice is prepared will be reflected on the subsequent month's <br />invoice. Such adjustments are limited to the three-month period prior to the most current <br />month for which the Contractholder provides eligibility data. <br />Contractholder agrees to bear the cost 75% of such Premiums without withholding or otherwise <br />charging Primary Enrollees for their coverage. Primary Enrollees agree to bear the remaining <br />25% of the cost of coverage for themselves and the entire cost of coverage for their enrolled <br />Dependents. <br />3.2 The Premium for each person electing continued coverage under the Continued Coverage <br />Option in Article 10 for himself or herself will be the same as that for a single Primary <br />Enrollee. The Premium for a person who also elects continued coverage for his or her <br />Dependents is the same as that for a Primary Enrollee with the same number of Dependents. <br />The Contractholder may charge persons choosing coverage under Article 10 such amounts as <br />are permitted by law. <br />3.3 This Contract is not in effect until Delta Dental receives the initial Premiums from the <br />Contractholder. <br />3.4 If this Contract terminates for any reason, the Contractholder agrees to pay all Premiums <br />earned by Delta Dental but unpaid by the Contractholder. <br />3.3 In addition to the amounts, if any, which Delta Dental withholds from payments to Dentists <br />as provided in Delta Dental Dentists Rules, the Contractholder authorizes Delta Dental to <br />deduct from each of its monthly payments to Delta Dental 14% of such amount as <br />compensation for Delta Dental's administration of this dental plan. <br />3.4 After the end of each Contract Term, the stabilization shall be calculated by Delta Dental from <br />the Effective Date of the Contract. The following percentage of any positive amount ("plus <br />stabilization") may be reflected in the calculation of the renewal rate for the succeeding <br />Contract Term and/or may be used to offset the additional cost of increased Benefits for the <br />succeeding Contract Term. <br />25% for Contractholders with an average monthly enrollment of 100 to 199 Primary Enrollees <br />50% for Contractholders with an average monthly enrollment of 200 to 299 Primary <br />Enrollees. <br />75% for Contractholder with an average monthly enrollment of 300 to 399 Primary Enrollees. <br />100% for Contractholders with an average monthly enrollment of 400 or more Primary <br />Enrollees. <br />Average monthly enrollment is based on the 12-month period preceding each renewal date of <br />this Contract. <br />Any negative or positive amount occurring during a Contract Term will be included in the <br />calculation of the stabilization during the succeeding Contract Term.
