(3) The days and time of entertainment activities to be held under <br />the permit; <br />(4) A statement that the applicant is the licensee of record, sole <br />party, or applicants are the sole party, either directly or indirectly interested in the <br />entertainment for which a permit is sought, and that no other person is, or will be, in any <br />manner interested, directly or indirectly, during the continuance of the permit; <br />(5) A statement of the ownership of the realty of the premises for <br />which the permit is sought; <br />(6) The name of at least one responsible party on the premises, <br />as well as a telephone number on the premises where such a person can be reached; <br />(7) Detailed description of all entertainment to be provided, <br />including, but not limited to, amplified music, dancing, and any other similar live <br />entertainment activities; <br />(OfM The skief of pel?se City Manager, or designee, shall investigate the <br />persons applying for such permit, shall consider the items and standards for granting set <br />out in section 44-5 11-6 and, in conformance therewith, shall determine which permit <br />category applies pursuant to section 11-7, and then either grant such permit if all of the <br />conditions of approval have been met. In the event that all of the conditions of approval <br />have not been met, the City Manager, or designee, may of deny the permit applied for, <br />and shall forthwith notify the applicant of 4is the action along with a statement identifying <br />the reasons for the denial. <br />(G)f!21 No person, firm, partnership, corporation, company, or non-profit or <br />charitable organization shall conduct any , <br />place entertainment, without first making application for and obtaining a permit so to do so, <br />nor shall any person conduct the same during the time while a permit so to do so is <br />revoked. <br />(d)ff Any modifications or <br />revisions to the live entertainment provided under an entertainment permit require City <br />Manager, or designee, approval. <br />(g) An entertainment permit shall be valid for a period not to exceed one <br />(1) year from date of issuance. Entertainment permits are not automatically renewed and <br />it shall be the responsibility of the permit holder to file a new application for an <br />entertainment permit at least forty-five (45) days prior to the previous permit's expiration. <br />(h) All establishments within the City that were conducting activities <br />requiring permitting and regulated under this section whose activities existed prior to <br />[Ordinance Adoption Datel, shall have a period of one (1) year from the date of the <br />75A-25
