Exhibit A SCOPE OF SERVICES— Nancy Lopez - Zumba Fitness A. Provider shall conduct Zumba Fitness class for participants aged 55+ B. Zumba Fitness class will be held one time per week for I hour/day, total of I hour/week. C. Class fee will be by participant DONATION. D. Classes shall be 1 day a week, June through August (Summer), September through December, (November and December shall be three-week sessions/Fall), January through March (Winter), April through June (Sprang). E. Provider shall work with City staff in setting a schedule for classes, including the location, specific days and hours when class will be held and holidays to observe. F. Provider shall provide material, supplies, equipment, records and personnel. Provider shall be responsible for cleanup of the facilities and materials and shall ensure the safety and effectiveness of instruction. G. 1f Provider allows others to teach his/her class, those teachers must be over 21, have obtained and maintain an instructor rating, and be covered by Provider's insurance. Provider shall provide City with documentation to verify instructor and insurance requirements. CLASS SIZE A. Each class must have a minimum of 5 students and no more than a maximum of 60. B. No registration will be accepted after the second meeting of class C. In the event the minimum number of enrollees is not realized by the second meeting of the class, the class shall be canceled and Provider will be under no obligation to provide services. CLASS FEES A. Each participant may pay a DONATION to Provider for class, no student will be refused to entry to class, except if class meets maximum. D. No refunds will be made to participants. C. Provider shall collect and retain all donations. D. Provider shall receive one -hundred per cent (100%) of the total donations collected each month. E. Provider agrees that City is entitled to audit Provider's records and classes to ensure compliance with this Agreement.
