E. INSURANCE <br />CMCMC will maintain the following levels of insurance and will require <br />that all of our subcontractors maintain the same levels of coverage. <br />Certificates of insurance shall be in duplicate and addressed to the City of <br />Santa Ana. All insurance shall remain in effect during the life of the <br />contract. Certificates must indicate that the City shall receive written <br />notice 30 days prior to any cancellation or reduction in coverage. <br />1, Comprehensive General Liability insurance to include Completed <br />Operations, Contractual, Independent Contractors and Personal Injury; <br />and Automobile Liability, including all auto; with at least the <br />following combined limits of Liability: <br />a. Primary Bodily Injury Liability limits of $1,000,000.00 per <br />occurrence. <br />b. Primary Property Damage Liability limits of $1,000,000.00 per <br />occurrence. <br />c. Primary Bodily Injury and Property Damage Liability combined <br />single limit of $1,000,000.00 is preferred. <br />2. Medical malpractice insurance of not less than three million dollars <br />($3,000,000.00) per occurrence. <br />3. Employer's Liability with a combined single limit of $1,000,000.00. <br />4. Worker's compensation insurance, in accordance with the provisions <br />of the Labor Code of the State of California. <br />5. No more than 20 days after notification of award, a separate additional <br />insured endorsement is required for part "1" above.
