A. RECEIVING SCREENING <br />CMCMC recognizes that the completion and review of the intake <br />screening form is a critical health encounter. Not only does this process <br />initiate the health record for the inmate and begin to organize on-going <br />health care, but also more importantly, it is intended to serve the <br />preventive health function of protecting the confined population from <br />infectious diseases through astute medical evaluation. <br />The intake screening form is completed by the Detention Officers and <br />includes the following: <br />A. Inquiry into: <br />1. Fast history of illnesses and medical conditions, <br />2. Known allergies, <br />3. Current illness and health problems, <br />4. Communicable diseases (including sexually transmitted diseases), <br />5. AlcohoUdrug abuse history (method/types, as well as date and time <br />of last usage), and problems related to stoppage, <br />6. Medications currently being taken and other special health <br />requirements, <br />7. Dental status, <br />8. Chronic health problems (including behavioral medicine), <br />9. Dietary restrictions, and <br />10. History of, and current possibility of; suicide risk. <br />B. Observation of. <br />1. State of consciousness, <br />2.
