Programs included those offered by the <br /> Mental Health Association of Orange County and the Santa Ana Police Department. <br /> <br /> 2. SUMMARY OF ACTIONS TAKEN DURING THE PROGRAM TO DEVELOP A CDC STRATEGY: <br /> Specific actions undertaken during the report period to assist the homeless through a system of <br /> care included the following: <br /> a. Actions taken to prevent homelessness - Santa Ana provided n meless service <br /> providers with ESG funds during the report period in order to z Santa Ana <br /> <br /> residents from becoming homeless. On a countywide basi `ilized to <br /> assist 70 persons with HIV/AIDS from becoming homeless. <br /> b. Actions taken to address emergency shelter - ESG fu ergency <br /> shelter and supportive services for the City's home re assisted <br /> during the report period with ESG funds. Grant ,iperating costs <br /> for emergency shelter programs and to prr homeless. On a <br /> countywide basis, HOPWA funds wer with HIV/AIDS with <br /> emergency shelter and supportive serv <br /> <br /> c. Actions taken to address tronsitio sed to provide transitional <br /> housing opportunities for 39 Sas x" L individuals were assisted with <br /> HOPWA-funded transitional' ,;X7ing the 2012-2013 report period, <br /> d. Actions t he needs o sub-populations. e.. those living on the streets and new <br /> funds such a r NOFA: Several homeless programs supported by the City of Santa Ana <br /> have an outreac omponent for special needs homeless sub-populations and the chronically <br /> homeless. These programs provide access to resources by referral and/or direct contact. These <br /> programs include: <br /> • Grandma's House, Human Options, Interval House, Laura's House and Women's <br /> <br /> Transitional Living Center - program efforts focused on victims of domestic violence. <br /> • WISE Place -served the needs of homeless elderly and young emancipated women. <br /> • Mental Health Association of Orange County, the Illumination Foundation and the Santa <br /> Ana Police Department worked directly with the City's chronic homeless population (i.e., <br /> <br /> 08/11/2013 20 <br /> <br /> 19E-28 <br />
