July 1, 2012 - <br />June 30, 2013 salary costs are based on total hours worked that includes <br />anticipated furlough hours for Workforce Specialist II and Fiscal <br />Specialist positions. <br />(2) Employee Benefits include health insurance, dental insurance, life <br />insurance, and long-term disability insurance. Also included are payroll <br />taxes such as FICA, Federal Unemployment Tax, State Unemployment Tax, and <br />Worker's Compensation Tax, based on the currently prevailing rates. <br />(3) Enrollments in WEX activities are contingent upon availability of funds <br />and shall consist of a minimum of thirteen (13) enrollments for the <br />period of March 7, 2012 through June 30, 2012; and forty (40) enrollments <br />for the period of July 1, 2012 through June 30, 2013. <br />(4) Benefits costs included are payroll taxes such as FICA, Federal <br />Unemployment Tax, State Unemployment Tax, and Worker's Compensation Tax, <br />(WAM0811A) <br />3of5 <br />(March 4, 2013) <br />
