b. <br />C. <br />• Round all hours worked to the half hour, no quarter hours or minutes. <br />• It is mandatory for participants to sign -in and out on Daily Sign -In Sheet <br />• Have one sign -in sheet per WEX participant <br />• Time cards and sign in sheets are collected at the same time. <br />• Emphasize the dates on the schedule in the supervisor handbook. The time cards <br />will be picked up over a two-day period. Most time cards will be picked up on <br />the first day of the pick-up dates. <br />• The participant will bring their first time card on the first day that they report to <br />work. Participant will give time card to supervisor. When the case managers <br />come to pick-up the time card they will bring a new one for the next pay period. <br />Emphasize the importance of keeping record of when the participants have <br />worked in case a question comes up later in the program. <br />Worksite Evaluation Form- is to be used as a guide to approve a worksite. <br />• Complete form during appointment. Go through each question with the <br />supervisor. <br />Ask to see the area where the potential participants will work. WEX staff will <br />evaluate each worksite for program compliance. <br />Always complete all the required paperwork Never immediately turn down a <br />site. If you have any concerns about a worksite, discuss them with your <br />supervisor before putting the site into the active or inactive files. <br />Request Form- is used by worksites to provide the details of theirjob openings. <br />• This must be filled out completely, accurately and neatly. This form is to be <br />used by the case managers to place participants into jobs Therefore the <br />information here must detail the type of job as well as any requirements that <br />may be expected of the participants <br />• A separate request form must be filled out for each position. Example: Some <br />sites will have clerical and custodial positions. There may be jobs in different <br />departments or with different supervisors. <br />• A detailed job description must be provided. (Attach additional information) <br />• Original form must be kept in file. Additional copies may be provided upon <br />request. <br />Agency Worksite Agreement- is the formal agreement between the worksite and <br />the City of Santa Ana. <br />Completely review the entire contract with employers. <br />Employers must sign on the last page of agreement after they completely <br />understand and agree to comply with program guidelines. <br />Keep all signed original copies of the agreement, supervisors are to keep a copy <br />for their records. <br />EXHIBIT A <br />
