Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The GHH Rapid Rehousing program will be developed in Santa Ana. Initial partners have been identified to <br />base the program on best practices, and to share resources and efforts to minimize costs and duplication. <br />Program implementation will include the following activities: <br />- Initial intake for Grandma's House clients will be derived from current clients, referrals from 211 and other <br />partners, and from community outreach efforts through the Nana's Kidz program. <br />- Following the intake, the client will work with the RHH Case Manager for housing seach and placement. <br />- Grandma's House will provide temporary transitional housing to qualified participants in the rapid rehousing <br />process. <br />- Grandma's House will then provide support services to their clients that are placed in housing. These services <br />will include Case Management, Access to the Computer Labs, Life skills development including financial <br />literacy, roommate relations etc..., workforce training and job search case management, referrals to <br />counseling, medical services and other participant needs. <br />- 8 Santa Ana households will be identified, placed, and provided access to support services. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
