Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The Illumination Foundation (IF) proposes to utilize ESG funding to rapidly re -house 8 households who have lost <br />heir housing and provide them with deposit assistance, rental assistance, and essential services to prevent them <br />from further spiraling into poverty. Funding is allocated according to HUD's HPRP guidelines. Funds are issued <br />directly to third -party members — not to the client. Rental assistance is provided as funds allow. Rapid - <br />Rehousing's guidelines are flexible and allow jurisdictions to allocate funding according to client need. Some <br />households may need a one -time infusion of assistance, while others may require longer -term assistance to <br />achieve stability. The following identifies the ranges for funding allocations. <br />:urity deposit assistance: <br />• Provide one -time deposit assistance payment for 2- 6 households at an average of $1,200 each ($2,400 - <br />$7,200) <br />trt-term rental assistance from 1 -3 months: <br />• Provide 2 -6 households with short term rental assistance at an average of $800 a month for 3 months <br />($4,800 - $14,400) <br />ssential services: <br />• Case managers assist clients through the application, intake, and housing search and placement <br />processes. Case managers and clients address risk factors leading to homelessness: low income /job loss, <br />mental illness, history of non - compliance, physical disabilities, credit problems, medical debt, pending <br />foreclosure of rental housing, legal problems, over - crowding, etc. <br />Other services include: life skills, workforce development, financial literacy, medical services, behavioral health <br />;ounseling, substance abuse counseling, and connections to social services and other community resources <br />vould be provided to households as appropriate. Case managers will develop at least 8 individualized <br />tevelopment plans for Santa Ana rapid rehousinq clients <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter.
