Total number of unduplicated households (Santa Ana and Non -Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12 -month contract period. <br />8 Households <br />II. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />Unemployment, loss of housing, and the economic climate are forcing an increased number of families, <br />particularly the very low- income, into homelessness. Interval House's Rapid Re- housing Program will offer <br />Rental Assistance and Housing Relocation and Stabilization Services to ALL eligible homeless populations in <br />Santa Ana: <br />1) Short- and medium -term rental assistance will be provided to individuals and families who are homeless. <br />Based on previous experience, it is anticipated that the average term of assistance will be 3 months at an <br />average rent of $1,100 per month, though the actual assistance provided will be based on the applicant's need. <br />2) Housing Relocation and Stabilization Services: Interval House will offer clients a full range of services under <br />the supervision of case managers, including: <br />Financial Assistance: May include funding the applicant's rental application fees, security deposits, first and <br />last month's rent and moving costs. <br />Housing Stability Case Management: Case Managers will work directly with clients on initial evaluation & re- <br />evaluation for the program; Individualized Service Plan; counseling; securing basic resources and mainstream <br />benefits; monitoring and supporting progress; and referrals. Case Managers will also coordinate: Housing <br />Search & Placement, Mediation and legal services as needed, and Credit Repair/ Financial Management. <br />Jcneaule or F'ertormance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
