City of Santa Ana <br />Scope of Work <br />Name of Organization Mercy House Living Centers <br />Name of Funded Program Homeless Prevention Program <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />32 Households <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month <br />contract period. <br />Households <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The Homeless Prevention Program will provide financial assistance services as well as case management <br />services to eligible clients, who would become homeless if not for assistance. Finnacial assistance activities <br />include short -term rental assistance and /or utility payments. Clients eligible to receive either form of financial <br />assistance will also be provided with eviction mediation and on -going case management by qualified Mercy <br />House Program Coordinators to ensure that each client maintains housing stabalization and self- sufficiency <br />after financial assistance has been expended. <br />Funding from the Santa Ana ESG program will be used to provide direct sercvices to Santa Ana residents in the <br />form of short -term financial assistance, as described above. Additionally, funding will be used to support <br />program staff salaries dedicated to case management as well as program staff communication expenses. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
