City of Santa Ana <br />Scope of Work <br />Name of Organization Mercy House Living Centers <br />Name of Funded Program Rapid Rehousing Program <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />50 Households <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month <br />contract . d. <br />L 4 Households <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The Rapid Rehousing Program will provide financial assistance and case management to homeless families <br />and individuals that will allow them to quickly move from off the streets or out of emergency or transitional <br />shelters into permanent housing. Financial assistance services would be determined on a need basis by each <br />client household and would include short-term rental susidies and rental deposits. <br />Clients determined eligible for financial assistance will also be provide housing search and placement <br />services. Program Coordinators will serve as liasons between tenants and landlords to help manage some of <br />the challenges and stigmas homeless individuals face in finding affordable housing. <br />Additionally, clients will receive on -going case management by qualified Mercy House Program Coordinators <br />to help clients meet their employment, budgeting, financial, and overall life skills goals as well as ensure <br />their housing stabalization and self- sufficiency after financial assistance is expended. <br />Funding will be used to provide direct services to Santa Ana residents in the form of direct financial <br />assistance as described above. Additionally, funds will provide a portion of program staff salaries dedicated <br />to case management and program staff communication (phones) expenses. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
