• The business must register for a Data Universal Number System (DUNS) <br />identification number. (See attached application) <br />• The business must fill out a Form W -9 (Request for Taxpayer Identification Number <br />and Certification) and submit with the application. (See attached application) <br />• This is a one -time reimbursement per business. <br />• Funding is not transferable and other restrictions may apply. <br />IV. Application Approval Process <br />• Upon receipt of application, City staff will verify its contents and check that all <br />necessary items have been completed and attached. <br />• Applicants will receive written notice of approval, or denial within 10 working <br />days of application receipt. If the application is denied, it is the applicant's <br />responsibility to recheck that all items have been included and resubmit the <br />application. <br />• The City's Executive Director for Connnunity Development will approve the final <br />application and forward to the Finance department for audit and request for <br />disbursement of funds. <br />• Applications will be time - stamped and processed on a first -come, first- served <br />basis. <br />• Due to the limit in funding for the program, not all businesses that apply will <br />receive the incentive. <br />V. Completion of Approved Business Development Trainine Workshop <br />The business owner must complete an approved business development training workshop <br />prior to the disbursement of rebate funds. To sign up for an approved training workshop, <br />please visit www.ocsbde.ore or call (714) 564 -5200. Applicants will not receive the <br />grant if the training workshop has not been completed prior to the submittal of the <br />application. <br />V1.
