10.10 Bereavement Leave. <br />A. An employee shall be granted up to three (3) working days' leave of six (6) hours <br />each without loss of pay in case of death of a member of the employee's <br />immediate family. Such leave is designated as bereavement leave. "Immediate <br />family" as used in this section is limited to: <br />1. Any member of the employee's household related by blood or marriage; <br />2. A parent, parent -in -law, stepparent, spouse, registered domestic partner, <br />child, stepchild, brother, stepbrother, sister, stepsister, grandparent, or <br />grandchild of the employee, regardless of residence; <br />Any other relative of the employee by blood or by marriage where it can be <br />established by the employee that as a result of such relative's death, the <br />employee's presence is required to handle funeral arrangements and /or <br />matters of estate. <br />B. An employee may use up to twelve (12) hours of additional leave charged to their <br />Personal Necessity Leave balance when authorized by the Department Head. <br />10.11 Industrial Leave. Any period of time during which an employee is required to be absent <br />from his or her position by reason of an industrial injury or industrial illness for which he <br />or she is entitled to receive compensation shall not be considered a break in continuous <br />service for the purpose of his or her right to salary adjustments or to the accrual of <br />vacation and seniority. <br />ARTICLE XI <br />11.0 EMPLOYEE INSURANCE <br />11.1 Health Insurance. Part Time Civil Service employees shall be eligible to participate in <br />the City's medical, dental and vision insurance programs only. <br />A. Effective as soon as practicable following approval of this agreement by the City <br />Council, the City will increase the contribution for Health Insurance to cover 75% of <br />the premium costs for the employee and any dependents. The contribution amount <br />will be set at the 2014 Ca1PERS rate for the lowest cost HMO or at the rate of the <br />lowest cost HMO for the Other Southern California region whichever is greater. <br />B. PTCS employees will be allowed to enroll in the City's Dental Insurance program. <br />The City will contribute up to 75% of the premium costs (for each tier of coverage) if <br />enrolled in the Met Life HMO plan or the lowest cost dental HMO Plan whichever is <br />highest; if enrolled in the Delta Dental PPO the City will contribute 100% of the <br />premium costs for employees enrolled in Single coverage. <br />27 <br />25C -29 <br />
