ATTACHMENT A 2013 -DJ -BX -1065 <br />Project identifiers: See JAG Identifiers (attached) <br />1. Computer hardware /software <br />2. Equipment - General <br />Budget and Budget Narrative: Outline how JAG fiords will be used to support and implement the <br />program. Include a breakdown of costs <br />Of the total award of $14,464 allocated to the city of La Habra, 10% ($1,446) will be directed to the <br />Orange County PROACT Taskforce, and 9% ($1,302) will be directed to the County of Orange for <br />administrative costs, leaving a remaining balance of $11,716. <br />An estimated $6,500 will be spent on COMPUTER SOFTWARE to purchase the Coplogic LEAPS <br />software; and the balance of the award, estimated at $5,000, will be spent on EQUIPMENT — GENERAL, <br />to purchase two AED's (vendor to be determined) which are estimated to cost $2,500 each. Should there <br />be unanticipated cost increases, then non - general fund monies will be used to supplement the grant <br />fimding. <br />Timeline and/or project plan: Identify when the goals and objectives will be completed <br />This project involves a relatively simple purchase and installation of computer software and it is <br />anticipated to be completed within 120 days of grant approval. <br />The goal is to purchase and install the software and equipment within 90 days of grant approval and <br />award, <br />Funding: <br />Total Allocation: $14,464 <br />PROACT Contribution: (10 %) $1,446 <br />Administrative Fee (9 %): $1,302 <br />Funds Retained by City/County: $1.1,71.6 <br />2013 -DJ -BX -1065 Page 21 of 29 <br />
