A. The City shall continue to pay one hundred percent (100 %) of the <br />premium cost for term life insurance coverage under the policy it maintains on behalf of <br />its officers and employees in order to provide each person designated as unrepresented <br />Executive Management (EM) with life insurance coverage in an amount equal to three <br />times such employee's annual rate of salary to a maximum of $300,000, provided said <br />affected employees can provide evidence of insurability for coverage above $150,000 if <br />so required by the terms and conditions of said term life insurance policy. <br />B. In the event any such affected employee is determined to be <br />ineligible for said insurance coverage, the City will attempt to provide as much coverage <br />as may be obtained at a reasonable cost without having to provide evidence of <br />insurability. <br />C. Affected employees shall have the option of redesignating <br />coverage of the City- sponsored term life insurance plan in the same manner as <br />provided for represented members of PMA. This amount may be applied to the options <br />within the City - established Cafeteria Benefit Plan." <br />Section 3: That except as amended by this Resolution, all other provisions of <br />Resolution No. 91 -066, as amended, shall remain in full force and effect. <br />Section 4: This Resolution shall be operative from and after May 1, 2014. <br />ADOPTED this 6t" day of May, 2014. <br />APPROVED AS TO FORM: <br />Sonia R.
