Exhibit A SCOPE OF SERVICES— Eliana Drew Provider shall conduct Ballet for Tots, Tiny Tot Tap and Tumbling for children 2yrs — 4yrs Adults and Ballroom Dance for children 16yrs - Adults A. Ballet for Tots class will consist of 4 weeks session, held 1 day per week, 1 hour per day- $28 per 4 weeks. B. Tiny Tot Tap and Tumbling class will consist 4 weeks session, 1 day per week, 45 minute per day -$28 per 1 week. C. Ballroom Dance class will consist of 4 week session, 1 day per week, I hour per week, per day -$28 per 4 weeks. D. Classes will be throughout the year. E. Provider and City staff shall mutually agree upon a schedule for classes, including the location, specific days and hours when class will be held and holidays to be observed. F. Provider shall provide materials, supplies, equipment, records and personnel. Provider shall be responsible for cleanup of the facilities and materials and shall ensure the safety and effectiveness of instruction. G. If Provider allow others to teach his/her class, those teachers must be over 21, have obtained and maintain an instructor rating, and be covered by Providers insurance. Provider shall provide City with documentation to verify instructor and insurance requirements CLASS SIZE 1) Each class must have a minimum of 6 paid students and no more than a maximum of 18. 2) If the minimum registration has not been reached by the second class, the class may be cancelled by mutual agreement of Consultant and City, without any compensation owed to consultant. 2) Consultant will provide and be responsible for equipment, records, and personnel and clean up of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. 3) If Consultant allows others to teach his/her class, those teachers must be over 21, have obtained and maintain an instructor rating, and be covered by Consultants' insurance. Consultant shall provide City with documentation to verify instructor and insurance requirements. CLASS FEES 1) Each participant shall pay $28/ 4weeks for each participant. Not to exceed $25,000.00 fiscal year. 2) The City shall collect registration from each participant during the registration period. Consultant shall collect material fees, and shall refer all interested participants to City for registration. 3) Consultant agrees that City is entitled to audit Consultant's records and classes to insure compliance with this Agreement. 4) City shall prepare class rosters and provide a copy to Consultant. Only registered participants may participate in class. 5). Consultant agrees that City representative shall be entitled to audit Consultant's records to insure compliance with this Agreement and that all participants are registered. 6). Consultant may not waive class participation/registration fees. 7). City shall prepare class rosters and provide a copy to Consultant. Only registered participants, listed on roster may participate in class.
