AMENDED ADDITIONAL SERVICES AT MEMORIAL CENTER: Amended agreement to include acting as Substitute for Memorial Center as follows: A. Provider will teach Fitness Dance class on a monthly basis for ages 10 + B. Provider will teach Zumba Gold class on a monthly basis for ages 13 + C. Fitness Dance class will be One class per day, Two days per week, $20/monthly, or $5 drop in fee. D. Zumba Gold class will be 1 class per day, Two days per week, $25, or $5 drop in fee. E. Zumba Gold class will be 1 class per day, One day per week, $15/monthly or $5 drop in fee F. The classes will be held throughout the year as agreed between Provider and City. CLASS SIZE A. Each class must have a minimum of 5 students and no more than a maximum of 21. B. No registration will be accepted after the second meeting of class C. In the event the minimum number of enrollees is not realized by the second meeting of the class, the class may be canceled by the Provider. Provider will be under no obligation to provide services and the City will have no obligations to pay Provider compensation. CLASSFEE A. Each participant shall pay a $25/$20/$15 or $5.00 drop in fee for registration fee per month. Anticipated revenue under this Agreement shall not exceed $25,000.00 annually. B. No refunds will be made to participants after the first week of class unless the class is cancelled by the City. C. The City shall collect registration fees from each participant during the registration period. Provider shall not collect fees, but shall refer all interested participants to City for registration. D. Provider shall receive seventy percent (70%) of the total fees collected each month. City and Provider agree that City shall retain thirty percent (30%) of the fees collected as an administration fee. E. Provider agrees that City is entitled to audit Provider's records and classes to insure compliance with this Agreement. F. Provider may not waive class participation/registration fees. G. City shall prepare class rosters and provide a copy to Provider. Only registered participants may participate in class.
