City of Santa Ana <br />Scope of Work <br />Name of Organization Grandma's House of Hope <br />Name of Funded Program GHH Emergency Shelter Program <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />30 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month <br />contract period. <br />99 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The GHH Emergency Shelter Program reserves beds for emergency use by 10 Santa Ana residents who are <br />uniquely challenged homeless women that may require services not available at traditional emergency <br />shelters. GHH's program is focused on 2 key groups: <br />- Survivors of Human Trafficking with specialized emergency services including case management, translation, <br />counseling specifically for their circumstances, life skills training, job skills training when appropriate and <br />placement in a safe, stable housing situation with family or a long -term program. <br />-To serve homeless women struggling with severe illness or treatment, in need of emergency shelter as they <br />locate stable housing for their recovery. GHH is also preparing to offer temporary housing for homeless <br />women in need of short -term hospice care. <br />Activities during the funding period will include providing the following services to 10 Santa Ana residents: <br />Safe Housing and Meals <br />Case Management <br />Creation of an Individual Action Plan <br />Group/ Individual Counseling <br />Mental and Physical Health Referrals <br />Job Search and Skill Training Support when relevant. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
