If the Medical Marijuana Collective is an unincorporated association, a <br />copy of the Articles of Association. <br />12. The name and address of the applicant's current Agent for Service of <br />Process. <br />13. A copy of the City's Acknowledgment of Medical Marijuana Collective <br />Operating Limits and Restrictions, listed in Section _._.050, containing a <br />statement dated and signed by each Management Member, under penalty of <br />perjury, that they read, understand and shall ensure compliance with the <br />aforementioned operating limitations and restrictions. <br />14. A copy of the City's Acknowledgment of Prohibited Activities, listed in <br />Section —. _.090, containing a statement dated and signed by each <br />Management Member, under penalty of perjury, that they read, understand and <br />shall ensure that neither the Collective nor its members and Management <br />Members shall engage in the aforementioned prohibited activities. <br />15. A statement dated and signed by each Management Member, under <br />penalty of perjury, that the Management Member has personal knowledge of the <br />information contained in the application, that the information contained therein is <br />true and correct, and that the application has been completed under the <br />supervision of the Management Member(s). <br />16.
