City of Santa Ana Scope of Work Name of Organization Orange County Association for Mental Health dba MHA Name of Funded Program City of Santa Ana Outreach Annual Accomplishment Goal I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the funded program, named above, during the 12 -month contract period. 20001 Persons II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month contract period. 500 Persons Program and Funding Description III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. What specific activities will be undertaken during the contract period. The primary goals of the program are to first locate, then engage the mentally -ill homeless of Santa Ana, and assess their social, mental health and medical needs. Secondly the Outreach Staff willl assist homeless consumers in accessing mental health, social services and the medical services they need to improve their situations and end their homelessness. A third goal will be to provide mental health expertise to the local police, business owners, other homeless providers in the city and the community as a whole to assist them in dealing with and assisting to end the homelesness of these indivduals. The Outreach Team will engage 500 unduplicated homeless individuals in the next 12 months and provide2000 units of services. Of the 500 unduplicated homeless individuals, 200 will be successfully linked to the medical, mental health, financial and social services they need to stabilize their symptoms,end their homelessness and result in a more successful and satisfying life. The funding through the City of Santa Ana ESG will fund a portion of the salary /wages /benefits of a bi- lingual /bi- cultural Spanish- speaking outreach worker to serve the City of Santa Ana exclusively.
