ARTICLE XII <br />12.1 EMPLOYEE INSURANCE <br />12.1 Health Insurance. The City shall contribute the following amounts toward the <br />payment of premiums for affected employees and their dependents under <br />the existing health insurance programs or new program or programs <br />providing substantially similar or improved coverage and benefits selected in <br />accordance with procedures in effect on the effective date of this <br />Agreement: <br />A. For each such employee who is covered under a spouse's non -City <br />sponsored health plan, the City shall pay the employee a cash <br />payment each month in an amount equal to 50% of the monthly <br />premium amount for the City's lowest "employee- only" coverage, if <br />said employee waives, in writing, City -paid coverage. <br />B. The City shall contribute toward medical premiums an amount <br />consistent with the rates then in effect for each tier of the CalPERS <br />Kaiser Permanente HMO Other Southern California plan. <br />C. Effective as soon as practicable following approval of the <br />Memorandum of Understanding by the City Council, CASA members <br />shall no longer be required to contribute forty ($40) dollars per month <br />toward the cost of medical insurance premiums. <br />D. Any contribution necessary to maintain benefits under said medical <br />plans in excess of the amount set forth above shall be borne entirely <br />by the employee. <br />12.2 Dental Insurance. The City agrees to contribute a maximum of up to ninety <br />($90) dollars per month per employee toward the payment of premiums for <br />dental insurance plans provided by the City for employees covered by this <br />Agreement and their eligible dependents. <br />A. Effective as soon as practicable following approval of the <br />Memorandum of Understanding by the City Council, CASA members <br />shall no longer be required to contribute ten ($10) dollars per month <br />toward the cost of dental insurance premiums. <br />B.
