Exhibit A <br />SCOPE OF SERVICES— Karina M. Martinez <br />Provider shall conduct Intro to Ballet for children 7-10 yrs and Primary Ballet for children 4-6 yrs <br />A. Intro to Ballet class will consist of 4 weeks session, held 1 day per week, 50 minutes per day- $20/4weeks <br />B. Primary Ballet class will consist of 4 weeks session, held 1 day per week, 50 minutes per day- $20/4weeks <br />C. Classes will be throughout the year. <br />D. Provider and City staff shall mutually agree upon a schedule for classes, including the location, specific days and <br />hours when class will be held and holidays to be observed. <br />E. Provider shall provide materials, supplies, equipment, records and personnel. Provider shall be responsible for <br />cleanup of the facilities and materials and shall ensure the safety and effectiveness of instruction. <br />F. If Provider allow others to teach his/her class, those teachers must be over 21, have obtained and maintain an <br />instructor rating, and be covered by Providers insurance. Provider shall provide City with documentation to verify <br />instructor and insurance requirements <br />CLASS SIZE <br />1) Each class must have a minimum of 4 paid students and no more than a maximum of 12. <br />2) If the minimum registration has not been reached by the second class, the class may be cancelled by mutual <br />agreement of Consultant and City, without any compensation owed to consultant. <br />2) Consultant will provide and be responsible for equipment, records, and personnel and clean up of the facilities <br />and materials necessary to ensure the safety and effectiveness of said instruction. <br />3) If Consultant allows others to teach his/her class, those teachers must be over 21, have obtained and maintain <br />an instructor rating, and be covered by Consultants' insurance. Consultant shall provide City with <br />documentation to verify instructor and insurance requirements. <br />CLASS FEES <br />1) Each participant shall pay $20/4weeks. Anticipated revenue from this class is $25,000.00 <br />2) The City shall collect registration from each participant during the registration period. Consultant shall collect <br />material fees, and shall refer all interested participants to City for registration. <br />3) Consultant agrees that City is entitled to audit Consultant's records and classes to insure compliance with this <br />Agreement. <br />4) City shall prepare class rosters and provide a copy to Consultant. Only registered participants may participate <br />in class. <br />5). Consultant agrees that City representative shall be entitled to audit Consultant's records to insure compliance <br />with this Agreement and that all participants are registered. <br />6). Consultant may not waive class participation/registration fees. <br />7). City shall prepare class rosters and provide a copy to Consultant. Only registered participants, listed on roster <br />may participate in class. <br />
