City of Santa Ana <br />Scope of Work <br />Name of Organization Orange County As: <br />Name of Funded Program City of Santa Ana Outreach <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />2500 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month <br />contract period. <br />750 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The primary goals of the program are to first locate, then engage the mentally -ill homeless of Santa Ana, and <br />assess their social, mental health and medical needs. Secondly the Outreach Staff willl assist homeless <br />consumers in accessing mental health, social services and the medical services they need to improve their <br />situations and end their homelessness. A third goal will be to provide mental health expertise to the local <br />police, business owners, other homeless providers in the city and the community as a whole to assist them in <br />dealing with and assisting to end the homelesness of these indivduals. The Outreach Team will engage 750 <br />unduplicated homeless individuals in the next 12 months and provide 2000 units of services. Of the 750 <br />unduplicated homeless individuals, 250 will be successfully linked to the medical, mental health, financial and <br />social services they need to stabilize their symptoms,end their homelessness and result in a more successful <br />and satisfying life. The funding through the City of Santa Ana ESG will fund a portion of the <br />salary /wages /benefits of a bi- lingual /bi- cultural Spanish - speaking outreach workerto serve the City of Santa <br />Ana exclusively. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
