City of Santa Ana <br />Scope of Work <br />Name of Organization <br />Name of Funded Program <br />House Temborary Shelter <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />150 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month <br />contract period. <br />24 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />Transitional Shelter and Supportive Services: Our core program provides 6 -9 months of shelter for 35 -45 homeless <br />families with children each year. Residents receive an apartment unit, fully stocked kitchen, basic housekeeping <br />supplies, and clothing. During residency, families are required to save at least 80% of their discretionary income to <br />help them build savings, learn life skills, and become debt -free. In addition to food and shelter, we also provide <br />critically needed supportive services, including case management, counseling, life skills development, a children's <br />program and employment and resource assistance. <br />Graduate Extension Program: Once a family is able to sustain itself, we help them obtain permanent housing by <br />providing resources, guidance, and assistance with the first month's rent or security deposit. All former residents can <br />continue to meet with their case manager, receive counseling and food, and participate in classes and workshops. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
