The parties entered into that certain Agreement # N-2014-093, dated May 13, 2014, <br />(hereinafter "said Agreement") by which Consultant has provided medical services for job <br />related injuries and illnesses. <br />B, The parties now wish to amend the Scope of services provided by Consultant under the <br />Agreement <br />WHEREFORE, in consideration of the covenants contained in said Agreement, and subject to all <br />the teras and conditions of said Agreement, except those amended in this First Amendment to <br />Agreement, the parties agree as follows: <br />1. Section 1, SCOPE OF SERVICES, shall be amended to include the following: <br />Consultant shall perform comprehensive pre-employment and miscellaneous <br />physical examinations, including: <br />a) For prospective City employees to perform the duties of the position for which <br />they are being considered (pre-employment assessments), using pre -determined <br />medical protocols for each job classification (such protocols may be modified by <br />the physician, in consultation with the City's Executive Director of the Personnel <br />Services Department, or his/her designee, as is necessary to make a determination <br />as to suitability for employment); and <br />b) Pre-employment drug screens (as required). <br />2, Except as hereinabove amended, all terms and conditions of said Agreement shall remain in <br />full force and effect. <br />IN WITNESS WHEREOF, the parties hereto have executed this First Amendment to Agreement <br />on the date and year first written above. <br />ATTEST: <br />MARTA D.
