Bosch, M.A. <br />May 1, 2014 <br />Fee Schedule and Costs <br />Hourly fee: $45 (except for vocational assessments) <br />Vocational Assessments charge: <br />15 or more (total) per calendar month: $200 - Full; $160 - Modified <br />14 or less (total) per calendar month: $225 - Full; $175 - Modified <br />The fees and charges for all services and costs would not exceed $90,000 in a fiscal year. <br />Potential activities and services for the purpose of or related to counseling and /or assessment <br />and testing qualifying for billed time includes: <br />• Direct customer contact, whether in individual, group, workshop, or Resource Center <br />settings, contact by telephone, casual encounter at the SAWC or at the job site when <br />related to assigned services <br />• Contact with customer fanuly, members of their support system, employer, or other <br />resources or professionals <br />• Researching, contacting, or arranging referrals or resources for outside services or <br />support not provided by myself or that is outside the scope of my license, training, <br />and /or experience <br />• Documentation and other administrative activities <br />• Communication with SAWC staff or outside resource persons <br />• Preparation time for direct client contact, such as review of files, handout development, <br />reviewing and /or obtaining reference or resource material, assessment/ testing material <br />acquisition and organization, material reproduction, and consultation with staff <br />• Onsite standby time (minimal) providing availability to walk -ins, and new clien t <br />introduction and scheduling <br />• Visitations to resources and. agencies utilized or potentially utilized in the service of the <br />customer <br />Reimbursement at cost would be expected for any material that might be obtained for customer <br />use, such as vocational assessment instruments, pamphlets, handouts, reference books, <br />videotapes, software, or other related resource material obtained with prior SAWC <br />management approval. <br />Exhibit A <br />
