F. Extension by Use of Vacation. After an employee's sick leave has been <br />exhausted, he or she may be granted permission to take any unused <br />vacation leave benefits he or she may have accrued. <br />G. Notice. The employee taking sick leave shall notify his or her immediate <br />supervisor either prior to or within four (4) hours after the time he or she is <br />scheduled to report for duty, or as otherwise established by departmental <br />regulations. When the absence is more than three (3) consecutive <br />working days, the employee must present to his or her Executive Director <br />a physician's certificate stating the cause of illness and that, in the <br />physician's opinion, the employee could not report to work because of <br />such illness or injury and that the employee is sufficiently recovered to <br />safely return to work. Such certificate shall be transmitted to the <br />Executive Director of Personnel Services with the report of the employee's <br />return to work. <br />A physician's certificate or other satisfactory written evidence of actual <br />illness or injury may be required after an absence of any duration less <br />than three (3) days. <br />H. Denial. No employee shall be entitled to sick leave with pay while absent <br />from duty because of sickness or injury purposely self - inflicted or caused <br />by willful misconduct; or, sickness or disability sustained while engaged in <br />employment other than employment by the City, for monetary gain or <br />other compensation, or by reason of engaging in business or activity for <br />monetary gain or other compensation. <br />Excess Usage. If sick leave is used in excess of that due and available an <br />employee, such excess sick leave will, first, be deducted from any <br />available vacation leave benefit; second, from the next scheduled salary <br />payment. <br />J. Necessity Leave. Each employee shall be afforded the opportunity to use <br />up to forty -eight (48) hours of sick leave per calendar year, on a non- <br />cumulative basis, as personal necessity leave. All of this personal <br />necessity leave may be used to attend to an illness of a child, parent, or <br />spouse of the employee. As used in this section, "child" means a <br />biological, foster, or adopted child, a stepchild, a legal ward, or a child of a <br />person standing in loco parentis; "parent" means a biological, foster, <br />adoptive parent, a stepparent, or a legal guardian; and "immediate family" <br />means any member of the employee's household related by blood or <br />marriage; a parent, parent -in -law, spouse, child, brother, sister, <br />Pis] <br />
