C. Procedure. <br />1. Upon receipt of a valid request for donations from an eligible <br />employee, the Executive Director or his or her designee shall post a <br />notice of the eligible employee's need for donation on bulletin <br />boards accessible to employees. No confidential medical <br />information shall be included in the posted notice. <br />2. Employee wishing to donate time to an eligible employee must sign <br />his or her authorization of the transfer of such donated time and <br />provide; <br />a. His or her name, department name, and employee number. <br />b. The number of hours of vacation time of the donation within <br />the limitation of Subsection 10.12(A)(3) above; <br />C. The name, department and employee number of the <br />recipient; <br />d. A statement indicating that the donor understands such <br />donation of time is irrevocable. <br />3. At the close of the donation period, the department shall verify that <br />each donating employee has accrued vacation time balances <br />sufficient to cover the designated donation. <br />4. The department shall submit all approved donation authorizations <br />for an eligible employee at one time for processing. No donation <br />authorizations for the eligible employee will be processed after this <br />period. However, employees who receive donations under this <br />procedure and who exhaust all donated hours may request an <br />additional donation period subject to the provisions of Subsection <br />10.12(A) herein. <br />5. The City shall add the donated time to the recipient's sick leave <br />account. <br />36 <br />
