A change in the form of business <br />organization may, at COUNTY's sole discretion, be treated as an attempted <br />assignment of rights or delegation of duties of this Agreement. <br />7.3 Real Property Disclosure: <br />If CONTRACTOR is occupying any real property under any agreement, <br />oral or written, where persons are to receive services hereunder, CONTRACTOR <br />shall submit the following information in addition to a copy of the lease, <br />license or rental agreement, as well as any other information requested, prior <br />to the provision of services under this Agreement: <br />7.3.1 The location by street address and city of any such real <br />property. <br />7.3.2 The fair market value of any such real property as such <br />value is reflected on the most recently issued County Tax Collector's tax <br />bill. <br />7.3.3 A detailed description of all existing and pending <br />agreements, with respect to the use or occupation of any such real property. <br />Such description shall include, but not be limited to: <br />7.3.3.1 The term duration of any rental, lease or <br />license agreement: <br />7.3.3.2 The amount of monetary consideration to be paid <br />to the lessor or licensor over the term of the rental, lease or license <br />agreement; <br />7.3.3.3 The type and dollar value of any other <br />consideration to be paid to the lessor or licensor; and <br />7.3.3.4 The full names and addresses of all parties to <br />any agreement concerning the real property and a listing of liens (if any) <br />(WRR0415) <br />9 of 39 <br />25C -11 <br />(April 7. 2015) <br />
