Subgrant Agreement <br />This Subgrant (hereinafter referenced as "Agreement ") is entered into between <br />Community Action Partnership of Orange County, a California non- profit corporation, <br />(hereinafter referenced as "Subgrantor" and City of Santa Ana, a municipal <br />corporation, (hereinafter referenced as "Subgrantee ") for the funding of Orange <br />County Partnerships to Improve Community Health Grant (hereinafter referenced as <br />"Grant ") for a one (1) year term and shall be effective on October 1, 2014. <br />Recitals <br />The Grant is authorized under Section 317(k)(2) of the Public Health Service Act, 42 <br />U.S. Code 247b(k)(2). The Grant Number is 1 U58DP005861 -01. The goals of the <br />Grant are (1) Create social and physical environments that promote good health for <br />all; (2) Promote health and reduce chronic disease risk through the consumption of <br />healthful diets and achievement and maintenance of healthy body weight; (3) <br />Improve access to comprehensive, quality health care services; and (4) Reduce <br />illness, disability, and death related to tobacco use and secondhand smoke <br />exposure. The Orange County Partnership to Improve Community Health project will <br />focus on goals one and two. The Grant aligns with national health objectives aimed <br />at creating social and physical environments that promote good health for all by <br />encouraging collaboration across sectors, implementing evidence -based strategies, <br />and measuring the effects of prevention activities. With the making of policy, <br />systems and environment (PSE) changes, the goal is to improve short- and long- <br />term health outcomes. Population -based strategies create greater access to <br />healthier environments where people live, learn, work, and play. The Grant reflects <br />the importance of cross - sector collaborations to build accepted and sustainable <br />changes and the need to evaluate and communicate what works. The PICH logic <br />model provides a framework to use throughout program planning, implementation, <br />and evaluation. All proposed objectives and activities in the work plan, known as the <br />Community Action Plan (CAP), should be connected to elements reflected in the <br />logic model and contribute to the ultimate goals of improving quality of life, averting <br />premature death and reducing medical costs. <br />1. Scope of Work <br />Subgrantee's scope of work is set forth in Exhibit "A" and is made part of this <br />Agreement.
