EXHIBIT A <br />I. SCOPE OF SERVICES - Blanca E. Arceo <br />A. Provider shall conduct Zmaba with Blanca class for age 12 years + <br />B. Zumba class is offered Monday through Friday, 1 hour, per day - $40/monthly, ages 12 yrs & up <br />C. Zumba class is offered Monday, Wednesday & Friday, I hour, per day - $30/monthly, ages 12 yrs & up <br />D. Zumba class is offered any two (2) days, 1 hour, per day, - $20/monthly, ages 12 yrs & up <br />B. Provider shall work with City staff in setting a schedule for classes, including the location, specific days and <br />hours when class will be held and holidays to be observed. <br />F. Provider shall provide materials, supplies, equipment, records and personnel. Provider shall be responsible <br />for cleanup of the facilities and materials and shall ensue the safety and effectiveness of instruction. <br />G. If Provider allows others to teach his/her class, those teachers must be over 21, have obtained and maintain an <br />instructor rating, and be covered by Provider's insurance. Provider shall provide City with documentation to <br />verify instructor and insurance requirements. <br />II. CLASS SIZE <br />A. Each class must have a minimum of 5 paid students and no more than a maximum of 60. <br />B. No registration will be accepted after the second meeting of class. <br />C. In the event the minimum number of enrollees is not realized by the second meeting of the class, the class <br />shall be canceled. Consultant will be under no obligation to provide services and the City will have no <br />obligations to pay Consultant compensation. <br />111. CLASS FEES <br />A. Each participant shill pay a $40/$30/$20 class registration fee per month five, three and two days a week. <br />Anticipated class revenue not to exceed $25,000 annually. <br />B. No refunds will be made to participants after the first week of class unless the class is cancelled by the City. <br />C. The City shall collect registration fees from each participant during the registration period. Consultant shall <br />not collect fees, but shall refer all interested participants to City for registration. <br />D. Consultant shall receive seventy percent (70%) of the total fees collected each month. City and Consultant <br />agree that City shall retain thirty percent (30%) of the fees collected as an administration fee, <br />E. Consultant agrees that City is entitled to audit Consultant's records and classes to insure compliance with this <br />Agreement. <br />F. Consultant may not waive class participation/registration fees. <br />G. City shall prepare class rosters and provide a copy to Consultant. Only registered participants may participate <br />in class. <br />
